What most descriptions of DHI leave out is how this works during surgery. It is not the surgeon who loads the pens. Loading is done by a team of two to three technicians working alongside the surgeon throughout the implantation phase.

The rotation works like this:

• The surgeon picks up a loaded pen from one side of the instrument trolley
• The pen is inserted into the recipient area at the correct angle and depth, the plunger is depressed, and the pen is set down on the opposite side of the trolley
• A technician picks up the used pen, reloads it with a fresh follicle, and returns it to the first side
• The surgeon picks up the next loaded pen and the cycle continues

Three pens are in rotation at any one time. This matters because the main vulnerability for a follicle during a hair transplant is time spent outside the body. By keeping the cycle moving continuously, each graft moves from extraction to placement with minimal delay. The surgeon never waits for a loaded pen; the technicians never wait for a used one. It is a tightly coordinated process.

DHI and FUE: Why They Are Not Competing Techniques

FUE, or Follicular Unit Extraction, refers to how follicles are removed from the donor area. A micro-punch tool is used to score around each follicular unit before it is lifted out. This extraction phase is called FUE whether the implantation that follows uses a Choi pen or pre-made incisions.

DHI refers to how the extracted follicles are placed into the recipient area, specifically via the Choi Implanter Pen rather than pre-made channels. The extraction that precedes it is always FUE. Every DHI procedure begins with FUE extraction. The two are sequential, not alternative.

One area where patients are sometimes misled is the concept of “no-shave DHI”. This refers to the recipient area only. Because the Choi pen does not require a shaved scalp to create incisions beforehand, the recipient area can often be left unshaved. The donor area, where the follicles are extracted, is always shaved regardless of which implantation method is used.
